The location lets us prepare our care team as well as determine cremation permit and death certificate costs based on local government fees.

This covers the cost of shipping and handling.

Please check this box if your loved one weighs between 250 and 500 lbs. The approximate weight is needed for proper care and transfer of your loved one. Please note that we cannot accommodate a person that weighs over 500lbs.

Please check this box if your loved one has a pacemaker. Our cremation process requires that we remove a pacemaker or other explosive device before cremation.

This covers the cost of shipping and handling.

Please check this box if you wish to view your loved one prior to cremation for identification purposes. This charge includes the professional preparation of your loved one for a 30 minute identification viewing at our facility for up to 10 people. Please note that the preparation does not include embalming, cosmetizing or other similar types of preparation.

Please check this box if the death has occurred at a residence and our staff must come to the residence. We charge this additional fee to provide an additional staff member to properly transfer your loved one into our care.

Illinois Death Removal & Transit Fee: our additional fee to transport the deceased to our facility when a death occurs outside of the State of Missouri, airfare and mileage is additional
